When an issue (Instruction) is assigned to you, you get a WhatsApp notification and an in-app notification. You then respond — adding a comment and (when required) a photo. You can respond from the mobile app or the web app, depending on your role. Responding is all the assigned user can do — the raiser's team accepts / rejects / closes.

Tapping the WhatsApp notification opens the mobile app (if installed) or the web app.
Log in with OTP or password — both work.
Phone-contact users (not registered in digiQC) log in via OTP and respond on the web app only.
Mobile app: open the Issue tab → To Do.
Web app: open the To Do tab → Issue.
The To Do list shows every issue assigned to you — respond one by one.
Add a comment (text) and a photo → submit.
The photo is mandatory only if the raiser raised the issue with a photo; otherwise it's optional.
Project users (e.g. Inspector, Project Admin) — web and mobile.
Auditor and Associate — web only.
Phone-contact users (unregistered) — web only (OTP login).
Your response goes to the raiser's team — they Accept (the issue is closed) or Reject (it comes back to you to respond again). (See Issue Handling Cases.)
